Dedicate a Name to Peace

For £100 have a name dedicated in the name of peace and the pity of war.

Your names given will be added to the online dedication wall and a QR code will be featured on the statue of the Pity of War to take viewers to this space.

To have a name or names that will be associated with the memorial, please either:
1a) use the PayPal button below to make your donation, then
1b) click on the link in the ‘Thank you’ page to return here, and
1c) complete the online form immediately below the PayPal button.
2) print out a form at the bottom and post it with a cheque (the address is on the form).

Please fill in the name of the dedicatee in BLOCK CAPITALS (this is simply to ensure we have the correct spelling). The actual dedication will appear in capitalized normal text, e.g. Joe Bloggs. If you want to dedicate more than one name, please add more below, as needed.
If you would like us to confirm the name as it will appear on the dedication, please provide your email address here.
If you want to add Gift Aid, please ensure you have ticked the boxes above, and enter your address in this box. Without this, and the date below, we cannot claim the Gift Aid from HMRC.
Please enter today's date.
If you would like to add any comment or message to us, please use this section. If you require a response, please ensure you have filled in the email box above!

Donating by Post
If you prefer to send your donation as a cheque by post, then please print out and complete a form, before posting them to us (the address is on the form). The form contains its own Gift Aid declaration.

A note on Gift Aid
Please notify us if you:
  • change your name or address whilst this declaration is still in force
• wish to cancel this declaration
• no longer pay sufficient tax on your income and/or capital gains

If you pay income tax at the higher or additional rate and want to receive the additional tax relief due to you, you must include all your Gift Aid donations on your self assessment tax return, or ask HM Revenue and Customs to adjust your tax code.

Please be assured that we will keep any information submitted private and never use it other than for administering the ‘Dedicate a Name to Peace’ process.
